The PDF form that will be generated should be printed on an A4 sized white bond paper. This form uses barcodes that needs a clean and crisp print, laser print is highly encouraged. If an inkjet is used, set the print quality to high and make sure there is no banding or streaking on the barcodes.

After printing the form 1) have it signed by your committee members and chair, indicating all the dates 2) the unit head, indicate the date 3) submit to the graduate school for evaluation and approval of the GS Secretary and the GS Dean.

THE GRADUATE SCHOOL HAS THE RIGHT TO REJECT FORMS THAT DOES NOT CONFORM TO THE STANDARD A4 SIZE, PRINT QUALITY, AND INCOMPLETE OR WRONG INFORMATION (SIGNATURES AND DATES).
