Maximum of 6 units will be credited for 100-level courses which could be placed either in major or minor courses

Courses numbered 190/191 will not be credited

Courses which are needed but not to be credited should be placed under "Other Courses"

Courses numbered 290/390 and 291/391 shall not exceed 4 units.

All courses below 100 level cannot be credited, put them under other courses.

The PDF form that will be generated should be printed on an A4 sized white bond paper. This form uses barcodes that needs a clean and crisp print, laser print is highly encouraged. If an inkjet is used, set the print quality to high and make sure there is no banding or streaking on the barcodes. Forms with erasures will be automatically rejected. Do not submit more than 1 original copy. Additional copy will be automatically disposed.

After printing the form 1) sign it, indicate the date 2) have it signed by your approved committee chair and members, indicating all the dates 3) the unit head, indicate the date 4) submit to the graduate school for evaluation and approval of the GS Secretary and the GS Dean.

THE GRADUATE SCHOOL HAS THE RIGHT TO REJECT FORMS THAT DOES NOT CONFORM TO THE STANDARD A4 SIZE, PRINT QUALITY, AND INCOMPLETE OR WRONG INFORMATION (SIGNATURES AND DATES).
